For my final Master's project, my group was tasked to explore the current pain points found across Aotearoa’s health landscape. Exploring opportunities where Tend could better their systems and services, setting them apart as both a business and a healthcare provider.
The result was the addition of My Health. A health plan, where patients and their health care team can communicate, plan and work collaboratively to proactively achieve better health outcomes for themselves and their whānau.

The Opportunity
Tend is a medical provider that champions patient friendly health services both online and in clinic.
Tend would like their patients to know about their past and future health encounters to encourage a more proactive approach to healthcare

After discovering mHealth opportunities that could be implemented at Tend we set out to reframe our design problem. Through group ideation and iteration we established a proposed design direction.

The result was the addition of My Health. A health plan, that stores patient’s Results, Repeat Medications, Referrals, Immunisation and General Family Health Timeline. Creating the opportunity to connect patients and healthcare teams so to create better health outcomes.

Our research indicated that most people are only interested in health concerns that impact them currently.
Presenting patient records for both Active and Long-term health concerns makes accessing the relevant and required patient information intuitive and fast.
Each individually concern is filed with the relevant timeline, results and any additional referrals for each concern.
Notification icons appear on the concern to indicate there is a task or up-coming task associated with that concern.
